All clients of the clinic are required to fill out a form containing a substantial amount of personal information — including their name, date of birth, SSN, and address. In many cases, this is accompanied by a photo of the client. Taken together, this effectively provides a ready-made dataset that could be used for identity theft and various types of fraud.
